These programs help older adults and caregivers navigate dementia, memory concerns, mobility challenges, medication management, and other age-related health issues.
Geriatric Assessment and Intervention Network (GAIN) Clinic
Provides specialized assessment and care for older adults with complex health needs, including memory concerns, mobility issues, medication management, behaviour changes, and caregiver support. Self-referral is available.

Provides dementia education, support programs, navigation assistance, and caregiver resources for people living with dementia and their families.
