About Us

Working together to improve healthcare in Peterborough and surrounding communities.

The Peterborough Ontario Health Team (POHT) brings together health care organizations, providers, patients, caregivers, and community partners to improve how services are planned, coordinated, and delivered across Peterborough City and County.

Serving Peterborough City and County

Our work reflects the unique needs of urban, rural, and Indigenous communities across the region.

The Peterborough Ontario Health Team serves communities across Peterborough City and County, including Curve Lake First Nation and Hiawatha First Nation.

Working together with healthcare and community partners across the region, POHT supports a population of more than 128,000 residents through connected, community-based care initiatives.

What is an Ontario Health Team?

Ontario Health Teams (OHTs) are groups of local health and community service providers that work together to organize and deliver more connected care to the community they serve.

Instead of organizations working separately, Ontario Health Teams bring together hospitals, primary care providers, home and community care, mental health and addiction services, community support services, long-term care, and many other partners to improve how people experience care.

Who We Are

The Peterborough Ontario Health Team (POHT) is a collaborative network of health care providers, organizations, patients, caregivers, and community partners working together to improve access to coordinated care across Peterborough City and County.

Our partners represent hospitals, primary care, home and community care, long-term care, mental health and addictions, Indigenous health services, community support services, and more.

Together, we are helping build a more connected, accessible, and person-centred health care system.

The Peterborough Ontario Health Team is guided by principles that help ensure our work is responsive, inclusive, and focused on improving health outcomes across the region.


Health Equity Lens

A Health Equity Lens is used to ensure that initiatives address barriers to care and improve access for all people, particularly those who may experience inequities within the health system.

By considering the unique needs and experiences of different populations, the POHT works to promote fair and inclusive access to services and supports.


Population Health Approach

A Population Health Approach focuses on improving the health and well-being of the entire community.

By understanding local needs, health trends, and the factors that influence health outcomes, the POHT can work with partners to identify priorities, improve services, and support healthier communities.
